We are a combination of a healthcare program and services agency designed to meet the special needs of patients and their loved ones in time of crisis due to a terminal illness, for which they do not elect aggressive treatment.

Attending Physician
Certifies the patient has a terminal illness and remains the patient’s primary physician working
as a member of the hospice interdisciplinary team.
Medical Director
Certifies the attending physician’s prognosis. Provides the primary link and involvement at every stage of the ongoing care of the patient. Provides consultation to the attending physician.
Skilled Nurses
Evaluates patient’s condition during regularly scheduled visits. Trained specifically to deal with pain and symptom management, coordinates the plan of care, and teaches the family proper patient care techniques and medication management.
Social Worker
Provides counseling, bereavement services and community referrals.
Chaplains
Available to provide spiritual support if needed to patient families and caregivers. Works as a liaison with clergy of the patient’s choice.
Certified Home Health Aides & Homemakers
Available to provide and assist with personal needs and care.
Volunteers
Available to provide companionship and assistance through visits, writing letters, legacy building, reading and shopping.
